set foreground colour to black or colour of choice, background null.
Preset shape heart or shape of choice. Normal line style, width 15. Draw to about the size shown.

Duplicate layer, image mirror. Then duplicate again and move up and slightly inward.
Duplicate again and image mirror.
Duplicate once more and move up and centre. you should now have this.

Magic wand click inside one of the shapes, Selections modify expand 6. Add one of your tubes as a
new layer and drop it below the shape layer you have selected and position (resize if necessary).
Ensure you are on the tube layer, invert selection and delete.
Repeat for the other 4 shapes with other tubes.
Add a drop shadow to each of these ten layers, I used V & H 2 opacity 69 and blur 4.
X out the background layer and merge visible.
Add your last tube and place where desired. Add a drop shadow ( I used V & H -4, opacity 60 and blur 10).
Preset shapes Ellipse, normal line , width 1, foreground colour any, background null.
Draw a large circle to enclose the top half of your shapes. go to text ( of choice) I used comic sans size 24.
Add you text of choice and position ( I use the space bar to do this).
In the layer palette click on the cross beside your vector layer, x out the ellipse and convert to raster layer.
T also added a layer above the background, flood filled it and added a mask, merged group and lowered the opacity.
